Seeking a Registered Nurse (RN)or Licensed Practical Nurse (LPN) to join our team in the Triage (Intake) office. This is a full-time, in-person, day shift, benefited position.

Job duties include receiving and directing the flow of patient referrals through the referral process, fielding caregiver and patient calls and scheduling visits and admissions.

The Triage RN/LPN is part of the interdisciplinary team. Job performance elements include record review for hospice and palliative care program entry criteria, coordination of admissions and follow up with case managers, and participation in the backup on call rotation schedule.

This caring, compassionate RN/LPN provides symptom management, teaching and guidance to hospice patients and their families. Wings of Hope Hospice provides care to patients wherever they live, in private homes, assisted living facilities, and nursing homes.

Wings of Hope Hospice & Palliative Care is a small non-profit jewel serving two counties and surrounding communities in rural southwest Michigan. We are a legacy hospice, formed in 1983 by a group of volunteers. Our community generously supports us as our work has gently touched the lives of many of our families, friends, neighbors and co-workers. A professional healthcare organization, Wings of Hope also has a warm family feel. Visitors to the office are sometimes surprised to hear so much laughter. Our medical director Dr. John Mulder is nationally known and recognized for his expertise as a certified palliative care physician. We are proud of the Torch Award for Ethics that we received from the West Michigan Better Business Bureau. We founded and are affiliated with the Wings Home, a 4-bed end of life care home that does not charge hospice patients room and board and where most of the caregivers are volunteers. We are growing and invite motivated and authentic individuals to join our clinical team.

Wings of Hope Hospice is seeking a Full time Certified Nurse’s Aide.
This caring, compassionate individual provides personal care for patients where the patient lives. Duties include bathing, oral hygiene, dressing and simple dressing changes. The aide works closely with a supervisory nurse and is part of the interdisciplinary team.
