Wings of Hope is seeking a social worker to be responsible for developing, implementing and managing the hospice and palliative care patients.

This includes but is not limited to identifying patient and family needs, connecting them with community resources, including grief support, and supporting the family systems. As part of the interdisciplinary team, this individual will educate and support hospice and palliative care staff, client and family/caregivers.
Wings of Hope Hospice provides care to patients wherever they live, in private homes, assisted living facilities, and nursing homes.

This position is full time, salaried with benefits, including health insurance, flex savings, retirement savings plan, life insurance and generous paid time off.

• Master’s Degree in Social Work from an accredited School of Social Work;
• One year of social work experience in a healthcare setting;
• Currently licensed to practice social work issued by the State of Michigan.

Wings of Hope Hospice & Palliative Care is a small non-profit jewel serving two counties and surrounding communities in rural southwest Michigan. We are a legacy hospice, formed in 1983 by a group of volunteers. Our community generously supports us as our work has gently touched the lives of many of our families, friends, neighbors and co-workers. A professional healthcare organization, Wings of Hope also has a warm family feel. Visitors to the office are sometimes surprised to hear so much laughter. Our medical director Dr. John Mulder is nationally known and recognized for his expertise as a certified palliative care physician. We are proud of the Torch Award for Ethics that we received from the West Michigan Better Business Bureau. We founded and are affiliated with the Wings Home, a 4-bed end of life care home that does not charge hospice patients room and board and where most of the caregivers are volunteers. We are growing and invite motivated and authentic individuals to join our clinical team.

Evening Shift – 5pm-10pm (7 shifts in a two-week period) includes some weekends
The evening shift position will also include being on call for week nights, 5pm-7am, weekends 7am-7am and holidays. The rotation will be every 6 weeks.
Our staff provides patient care for terminally ill guests at the Wings Home in Allegan. (Certified Nurse’s Aide Welcome!) If you would like to work in a comfortable home with a maximum of four patients at a time, with support of Wings of Hope Hospice nurses 24 hours a day, this is the job for you.

This caring, compassionate caregiver provides personal care to patients including oral hygiene, changing briefs, simple dressing changes, assistance with taking medications and changing positions. Other duties may include light meal preparation, laundry and other household tasks.
